Log Message:

* removed the `ctx->full_rekeying' mechanism to make the code conform to 
  the API.
* moved `ctx->nlfsr_offset = 0' from `ECRYPT_keysetup' to `ECRYPT_ivsetup'.
* corrected a few bugs in the buffering mechanism.
